Purpose: This research aimed to explore the effects of the mandated Comprehensive Care for Joint Replacement (CJR) payment model on the utilization of occupational therapy (OT) in skilled nursing facilities in the state of Florida. The mandated CJR payment model was implemented for hospitals in 67 metropolitan statistical areas within the United States starting April 1, 2016. The CJR payment model was mandated by the Centers for Medicare and Medicaid Services (CMS) in an attempt to reduce overall spending for total joint replacements over a 90-day episode of care. This study sought to explore whether there was a change in OT utilization by Florida Medicare beneficiaries in skilled nursing facilities during the CJR implementation period.

Method: A retrospective data review was completed utilizing publicly available data from the CMS website. The research team conducted an independent sample t-test to compare OT utilization before and during the implementation of CJR in the mid-to-late 201Os, to determine if a significant change occurred between the two time periods.

Results: Upon conducting the independent samples t-test, our results (p = .468), indicated that there was no statistically significant difference in OT service utilization between the pre-CJR (2014-2015) and duringCJR time periods (2017-2018).
