Abstract
To help pharmacists advocate for integration of their patient care services in the health care team, Nova Southeastern University College of Pharmacy formed the Accountable Care Organization Research Network, Services, and Education (ACORN SEED). ACORN SEED offers pharmacy services at multiple primary care clinics in south Florida and is recognized by the Agency for Healthcare Research and Quality as a practice-based research network.
