Abstract

The management of medical conditions in the frail elderly who reside in post-acute and long-term care facilities (PALTC) can be challenging due to multimorbidity, progressive functional decline, psychosocial issues, and reduced life expectancy. This chapter will review some of the common clinical conditions that frequently occur in patients in the post-acute and long-term care continuum. The treatments prescribed by practitioners and treatment goals often differ from those of patients in other settings, and thus require an approach that balances the risks/benefits of treatment guided by clinician discussions with the patient and/or surrogate decision-maker.
