Abstract
Background: In today’s health care environment, the financial realities play an important role in health services. Patients often struggle in dealing with issues such as complex insurance policies, medical billing, high pill burden, poor medication insight and adherence. As medication utilization and expenditures increase in the United States, controlling medication related problems becomes increasingly important in creating a healthy nation. Objective: This project aims to incorporate the pharmacist's expertise and develop a comprehensive and state of the art pharmacist advocate program (PAP) call center to achieve desired therapeutic goals for patients by promoting safe, appropriate, and cost-effective medication use. Methods: The PAP call center will be located at the Nova Southeastern University’s College of Pharmacy that is one of the few colleges in the US to operate both a full-service community and institutional pharmacy. All members within the collaborative healthcare plans are eligible to take part in the study. Verbal informed consent will be obtained during the phone call. A trained clinical pharmacist and community pharmacy resident will provide the following interventions at the call center including: medication therapy management, medication adherence program, prior authorization assistance and formulary management. Results: It is expected that the PAP center will empower patients to actively participate in their own care while maximizing cost-effectiveness of the medications and improving healthcare outcomes.
