Abstract

Objective: Dimensional changes following extractions are inevitable. Many techniques and combinations of materials have been studied and shown to be effective at minimizing these changes. Although no material has been able to fully preserve the ridge, xenografts have been shown to be the most effective at maintaining the volume, however, the use of DFDBA results in the most vital bone when evaluated histologically. These factors are important when planning for a successful future implant supported prosthesis. A new biomaterial that combines a xenograft (DBBM) with DFDBA has recently been introduced to the market. The aim of the study is to compare the dimensional and histological changes following flapless ARP in the premolar region with a new material that combines DFDBA and DBBM to a commonly used mineralized allograft. Methods: the randomized clinical trial was performed at Nova Southeastern College of Dental Medicine Post-Graduate Periodontology Clinic. Twenty-six patients requiring premolar/molar extraction with future implant placement were randomly divided into two groups, one test and one control. Following extraction, the patients received either allograft or graft combination of DFDBA and DBBM. The changes in horizontal and vertical dimensions of the alveolar ridge as well as the histological changes were assessed and compared from four months to baseline. Results: The results showed no statistically significant differences between groups for horizontal and vertical changes at any of the timepoints (all p > 0.05). Even though one core biopsy was analyzed in the test group and four in the control group, bone percentage and vital bone percentage was lower in the test specimen at four months post-extraction. Conclusions: This study demonstrates that there is no statistical significance between the DBBM + DCP group when compared to allograft group for flapless alveolar ridge preservation in molar and premolar sites in both horizontal and vertical dimensional changes. A larger sample size and longer healing time may yield different results and provide better understanding of the healing process histologically.
